Request for Spouse/Dependent ID card must be completed by the RPI community member. The pickup of actual card can be either the community member that requested the card or the cardholder themselves. Spouse Card
Applicants must upload legal photo identification, such as a US Driver's License or Passport, and one legal document that indicates their spousal relationship must be submitted in the request.
Acceptable documents are:
- Marriage License with official seal
- I-20/J2 Paperwork
- RPI Employee Health Insurance Card
Dependent Card
Applicants for Dependent Cards must be 14 -20 years of age, must have legal photo identification, such as a US Driver's License or Passport. Employee must submit all forms for their dependent and must attach the dependent’s legal form/document that indicates a guardian and dependent relationship and date of birth.
Acceptable documents are:
- Birth Certificate with parents/guardians name on it.
- I-20/J2 Paperwork
- RPI Employee Health Insurance Card with dependent name on it
Individuals age 21 to 24 only qualify if enrolled as a full-time student; must provide proof of school enrollment (example: Letter from school’s Registrars Office on official letterhead/ Enrollment Verification Certificate www.studentclearinghouse.org)
Photo Guidelines:
- Must look like a driver's license (do not use a photo of your driver's license) or passport photo.
- Headshot only — from just above your head to your collarbone.
- Background must be plain white — no furniture, decorations, or scenery.
- No filters or effects.
- Face forward with your eyes open and a neutral expression or smile (we love smiles!).
- Wear everyday clothes. No hats or sunglasses.
- Head coverings for religious reasons are okay.
- Only you should be in the photo — no friends, pets, or objects.
